Financial Eligibility |

Income at or below 250% of the current Federal Poverty Level.

Not eligible for Medicaid.

No third party insurance that pays for more than 50% of the cost of covered medications.

Medical Eligibility |

HIV+ with proof of Western Blot. Verifications must be submitted with the application.

Pregnant women and their newborns (up to 6 weeks of age) may be eligible for AZT ahead of waiting list.
-
Children will get ARV and pcp prophylaxis ahead of waiting list.

Annual recertification required.

Enrollment |
Program currently capped - There is a waiting list. Local CARE Consortiums provide extra coverage for medications, home care and nutrition. |

Pharmacy |
Central pharmacy distributes medications. Physician faxes or mails approval letter and prescription to central contracted pharmacy. Medication is transported to HIV/AIDS clinic or, upon request, mailed to client. |

Nutrition |
Available through public health services Local CARE Consortiums. Call (334) 206-5364 for referral. |
